John Terry scored the first goal for Chelsea in their 4-3 win over Watford at Stamford Bridge on May 16.
The 36-year-old has failed to be involved in first-team play for the Blues since the arrival of new manager Antonio Conte in the summer.
The English centre-back made only 13 appearances for Chelsea this season.
NAIJ.com reported that the skipper announced he will be leaving the London club at the end of the season after 22 years.
He said after the game against the Hornets: “It’s a dream ending for me, to have the chance to celebrate my fifth Premier League title at the end of my last season as a Chelsea player.”
He was spotted with his wife Toni Terry enjoying a meal at Marias beach restaurant in Portugal after the game against Watford.
